Set against the backdrop of the Cold War era, the "white rats" were involuntarily selected by the Murkoff Corporation to test advanced methods of brainwashing and mind control. In a world of suspicion, fear, and violence, your ethics will be challenged, your endurance tested, and your spirit crushed – all in the name of progress, science, and profit.

You can complete the tests alone or in groups of 2, 3, or 4 players. Although the cooperative mode is not mandatory for the main missions, working together will provide a great advantage. No matter how many people play, the ultimate goal is to survive and escape.

Your mission is to complete Murkoff's therapy to earn the right to be released back into society. To do this, you must pass the Trials – deep story-driven therapies, and the MK-Challenges – shorter therapies taking place in redesigned areas of the existing map.

According to the pure style of Outlast, the core gameplay revolves around avoiding enemies, hiding, and trying to escape. Murkoff will provide tools to enhance stealth capabilities, create opportunities to escape, slow down enemies, and many other features. However, you have to strive to earn those tools and improve them.

The tool will allow you to stun enemies, blind them, see through walls, heal yourself and those around you. Murkoff will require you to carry out terrifying tasks in each trial. Only after completing these tasks will you be allowed to escape.

The game allows character customization – create your own test patient. The cell is your home during therapy, you can decorate it as you like. Choose tools, upgrades, and perks that suit your play style.

Collecting documents and items will help prove Murkoff's wrongful conduct towards patients. The testing process consists of three stages: Breaking down the self (attacking the basic belief system), Building the self (introducing a new ideology), and Redemption (completely abandoning the old ideology and accepting the new order).
